Remote Patient Monitoring (RPM)
Remote patient monitoring (RPM) uses sensors on the person and in the environment to collect medical and other forms of health data from individuals and electronically transmit that information securely to health care providers in a different location for assessment and recommendations. The Zanthion system establishes baselines for all event data such as heart rate and weight so that healthcare providers can provide appropriate care and reduce readmission rates.
Monitoring programs can collect a wide range of health data from the point of care, such as vital signs, weight, blood pressure, blood sugar, blood oxygen levels, heart rate, and electrocardiograms. Currently Zanthion collects heart rate through our wearables and blood pressure and other measurements through external devices. We are working hard to integrate as much sensory input as possible through our wearables such as skin temperature, blood oxygen, and blood pressure.
Zanthion data is then transmitted to health professionals directly to mobile devices and to aggregation services that present data in a way that makes it easy to understand for monitoring centers in primary care settings, hospitals and intensive care units, skilled nursing facilities, and centralized off-site case management programs. Health professionals remotely monitor patients through the use of our platform and act on events received as part of the treatment plan.

Medicare Reimbursement for CPT 99453, 99454, 99457
The Zanthion SMART RPM Solution supports new Medicare “Chronic Care Remote Physiologic Monitoring,” so that you can more effectively and efficiently remotely monitor and manage your patient care needs, including chronic care management. Our solution ships configured for your patient to their home. The Zanthion SMART Watch starts working the minute they turn it on transmitting real time health data to our comprehensive dashboard with aggregated data for the day, the week, and the month on heart rate and activity data.
- CPT 99453 offers reimbursement for the work associated with onboarding a new patient onto a RPM service, setting up the equipment and educating the patient on using the equipment. The average national Medicare payment for these services is $19.46. All you have to do is enter their name, address, and your medical tracking number into our system and we do the rest.
- CPT 99454 offers reimbursement for providing the patient with a RPM device for a 30-day period. Note that 99454 can be billed each 30 days. The average national Medicare payment for these services is $64.15. The Zanthion platform takes care of the requirements of supplying daily recordings and programmed alerts.
- CMS will reimburse for clinical staff time that contributes toward monitoring and interactive communication which includes phone, text and email. The average national Medicare payment for these services is $51.54 (non-facility). Our system comes with a client with immediate access through an icon for all patients and a follow up on all incidents with an assessment. These activities are rolled up into reports that you can easily share with Medicare for reimbursement.
